Golden Buckeye Card Fact Sheet

Lost card – to get a replacement card call 1-866-923-7879.

I have a Golden Buckeye card, how do I get prescription savings? – If you already have a Golden Buckeye card and want to receive prescription discounts, please call 1-866-923-7879 to receive a card with Ohio’s Best Rx.

 
Eligibility for the Golden Buckeye card – Ohio resident, 60 and over OR 18 or over with a permanent and total disability as defined by Social Security Administration.
 
How to get a new card if age 60 or over – if you have an Ohio Driver’s License or Ohio ID card you will get the Golden Buckeye card automatically through the mail around your 60th birthday.
If your address has changed please call 1-866-923-7879 and update your mailing address to get your new card. The Ohio’s Best Rx prescription discount card information is on the reverse side of the new Golden Buckeye card.
 
If you do NOT have a current Driver’s license or ID – take proof of age and apply in person at most senior centers and public libraries (call first) and complete an application in person.
You will receive a temporary card at sign up and you should receive your permanent card in approximately 3 weeks.
 
How to get the Golden Buckeye card if disabled – apply in person at most senior centers and public libraries (call first). You will need proof of age and a Medicare card OR SSDI acceptance documentation OR a copy of a document issued by any state dept. or public employee retirement system certifying permanent and total disability. Those are the only documents that will be accepted.
How soon can I use the card? – use it immediately. If you applied for a card in person and received a temp card, you can use it at MOST businesses. You can NOT use it at the state parks or pharmacies, you must have a current, hard plastic card. For the state parks, the card can be no older then four years. When you get the permanent card, you may begin using it at participating pharmacies for prescription discounts and state parks.
 
Who honors the card? – Nearly 18,0000 merchants and businesses in Ohio. Look for the "Golden Buckeye Honored Here" decal. If you’re not sure, ask if the business honors the card or offers savings/discounts for seniors, if interested in participating in Golden Buckeye the business may call 1-800-422-1976 to sign up. To find a listing of participants log on to www.goldenbuckeye.com
 
Can others use my card? – NO. If someone is eligible for the card, have them apply for their own.
